Step-by-Step Instruction

Step 7 – Enter Label for information field. For example Birthday Child
Step 8 – Click Show for drop down menu to select type of input field you want. For example date, text, check box, and more5.5 03
Step 9 – Enter a Value to prompt the correct input from customer. For example, Name of birthday star here. This will disappear upon being filled in by customer
Step 10 – Check box to make information required or leave blank
Step 11 – Click Save
